Decatur County Memorial Hospital is asking all Decatur County (DCMH) residents to fill out a quick, six-question survey regarding the health needs of our community.
This community health needs assessment takes less than five minutes to complete, but offers us a great wealth of information concerning our county and its needs.
The results will reveal where DCMH, as a hospital, needs to adjust its focus and offered services, so we can better serve Decatur County. The more people that complete the survey, the more accurate a view it will provide of what our community needs. It’s very important that you share what you think needs done regarding public health topics, so DCMH can put new plans and programs into action.
You can complete the survey two different ways. You can take the survey online, by going to: www.dcmh.net and clicking on the “News” tab (the survey link is in the article titled “DCMH Needs Your Help”). If you do not have internet access, paper surveys can be picked up and completed at the volunteer desks located at both main hospital entrances. All surveys must be completed by June 21.
